INCORPORATED COMPANY

In business for two years or less:
- commercial application, duly completed and signed
- "self-employed" application (with personal balance sheet)
- bank statements for the past three months*

In business for over two years:
- commercial application, duly completed and signed
- financial statements (if application exceeds $20,000)
- bank statements for the past three months*

  

SOLE PROPRIETORSHIP

In business for two years or less:
- commercial application, duly completed and signed
- "self-employed" application (with personal balance sheet)
- bank statements for the past three months*

In business for over two years:
- commercial application, duly completed and signed
- "self-employed" application (with personal balance sheet)
- financial statements (if application exceeds $20,000)
- bank statements for the past three months*
